Medical Assistant, Complex Care – Job Description

Employment Type: Full Time, Non-Exempt
Location: Rosedale Clinic | 2401 Fairview Ave N #145, Roseville, MN 55113
Shift Information: 10-hour shifts, 4 days per week (Monday - Friday)
Compensation: Starting at $27.00/hour, based on experience

About the Medical Assistant, Complex Care Role:

The Medical Assistant, Complex Care will work closely with our Advanced Primary Care Intensivist, Geriatrician and interdisciplinary team to provide clinical, clerical, and technical support for highly complex patients and consultative geriatric care. This Medical Assistant will also support seamless coordination of patient care, ensuring timely communication, follow-up, and collaboration across the care team. They will play an integral role in the success of Herself Health’s Memory Care Program, along with the management of frailty, dementia, and multiple chronic conditions of the patient panel.

Core Responsibilities:

  • Provide clinical support during geriatric consults, assisting with cognitive impairment screenings and assessments
  • Assist in the coordination and documentation of the Memory Care Program, supporting patient assessments, follow-up scheduling, and care plan updates
  • Room patients efficiently while modeling our “Her, first” values
  • Obtain and record vital signs and patient information accurately in the EMR
  • Prepare patients and assist providers during exams, procedures, and treatments
  • Perform phlebotomy, point-of-care testing, and administer injections as ordered
  • Review and update patient medication lists and prepare orders for provider review
  • Support patient and caregiver education around disease progression, care planning, and follow-up needs
  • Conduct proactive outreach for chronic disease management, medication adherence, and preventative screenings
  • Manage clinical supplies and equipment inventory, ensuring readiness for all visits
  • Provide empathetic communication and coordination with caregivers and family members
  • Protect patient confidentiality and ensure compliance with HIPAA standards
  • Collaborate closely with interdisciplinary team members (Physicians, NPs/PAs, Care Coordinators, Behavioral Health, and Operations)
  • Float to all five (5) clinic locations

Core Qualifications:

  • Completion of an accredited Medical Assistant Certification Program (diploma) or active Medical Assistant certification (required)
  • Phlebotomy training and competency (required)
  • Background in chronic disease management and care coordination
  • Experience supporting geriatric, dementia or complex patient populations
  • Strong interpersonal and communication skills, with a focus on patient and family engagement
  • Ability to maintain accuracy, organization, and composure in a fast-paced setting
  • Demonstrated professionalism, reliability and discretion with sensitive information
  • Proficiency in EMR systems

Additional Qualifications:

  • Passage of pre-employment background check and drug screen
  • Proof of U.S. Citizenship or eligibility to work in the United States
  • Compliance with all Herself Health vaccination requirements for patient-facing roles
