About the Role

We’re looking for a part-time Care Manager to join our team and help guide patients through their Paloma Health journey. This isn’t a health coaching role, although health coaches are welcome to apply. Instead, this is a patient-facing, emotionally supportive role for someone who loves connecting with people and helping them feel heard, understood, and empowered.

You’ll help patients understand their care plan, navigate labs, doctor visits, and medication refills, and feel motivated to take charge of their health—especially when they’re overwhelmed or discouraged. You’ll also assist with support tasks like password resets, triaging inbox messages, and resolving common patient requests.

What You’ll Do
  • Be the go-to contact for patients, helping them understand and move through their Paloma care journey
  • Offer encouragement, clarity, and practical support to women navigating thyroid, menopause, and weight changes
  • Coordinate behind-the-scenes tasks like lab order tracking, appointment follow-ups, and prescription support
  • Use our EHR to document patient interactions, manage requests, and support the clinical team
  • Help with light customer support, triage messages, and manage tasks like password resets and refund requests
  • Collaborate closely with clinicians, operations, and engineering to improve the patient experience
What We’re Looking For
  • Someone who loves talking to patients all day and helping them feel seen
  • Experience working in a healthcare setting (virtual care, clinic, back office, or EHR support is a big plus)
  • Familiarity with EHR systems and understanding how e-prescriptions and lab orders work
  • Empathy, patience, and great communication skills
  • Health coach certification is a plus but not required
Bonus if you have experience with:
  • Customer support platforms like HelpScout or Zendesk
  • Athena or similar EHRs
  • Hormonal health, autoimmune conditions, or thyroid-related care
  • Supporting midlife women’s health concerns
  • A health coaching certification (NBC-HWC or similar)
