About the Role
The Patient Navigator position is an exciting opportunity for compassionate, service-minded individuals who want to make a meaningful impact on patients' lives. In this role, you’ll serve as the primary point of contact for patients, guiding them through their fertility journey and working closely with a multidisciplinary team to ensure exceptional care and support.
We are seeking a Patient Navigator to join our dedicated team at IVF1 in Naperville, IL. This is a full-time position requiring availability Monday through Friday, from 8:00 AM to 5:30 PM, with 1-2 weekends per month.
Key Responsibilities:
- Serve as the primary contact for patients, providing guidance through treatment plans and education on procedures, testing, and follow-up care.
- Coordinate and schedule patient appointments, tests, and referrals to ensure continuity of care.
- Assist patients in understanding their medical care, insurance coverage, and financial resources.
- Communicate with healthcare providers, clinical staff, and administrative teams to ensure seamless care coordination.
- Maintain accurate and up-to-date patient records, including documentation of interactions and care plans in the electronic medical record (EMR) system.
- Provide emotional support to patients and their families, addressing concerns and removing barriers to care.
- Participate in team meetings, case conferences, and quality improvement initiatives to enhance care delivery.
- Educate patients on treatment protocols, self-care strategies, and medication usage.
- Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans.
- Advocate for patients’ needs and assist in resolving any issues or barriers to care.
- Perform other duties and projects as assigned.
Position Requirements:
- Education: High school diploma required. Bachelor’s degree in healthcare, social work, or a related field preferred; Certified Medical Assistant Certification (CMA) preferred.
- Experience: Minimum of 2 years in customer service, patient navigation, case management, or a related healthcare role.
- Skills:
- Strong knowledge of healthcare systems, insurance processes, and patient advocacy.
- Excellent communication, organizational, and problem-solving skills.
- Proficiency in using electronic medical records (EMR) and healthcare software.
- Empathy and a patient-focused approach, with the ability to work collaboratively in a multidisciplinary team.
- Bilingual skills are a plus.
Compensation & Benefits:
- Hourly Rate: $24.00 - $26.00 per hour (final offers based on experience, skills, and qualifications).
